It offers physical and behavioral health services together to treat all aspects of your health care needs including doctor visits, hospitalization, prescription drugs and services specific to CRS conditions. AHCCCS Complete Care Members with Children's Rehabilitative Services (CRS) Designation. Some AHCCCS Complete Care Members have been diagnosed with a CRS condition and are designated as CRS. Members who are designated CRS may receive specialized care. You can choose your own doctor and get extra programs that only UnitedHealthcare Community Plan provides.

What is Subscriber ID on insurance card UnitedHealthcare?This number is the last 5 digits of your member ID with 3 or 4 preceding zeros. For example, if your member ID is 680012345, you would enter the subscriber ID of 00012345. If your member ID is 6800012345, you would enter the subscriber ID of 000012345.
What is group ID on insurance card?Group number: Identifies your employer plan. Each employer choses a package for their employees based on price, or types of coverage. This is identified through the group number. If you purchased your insurance through the health exchange you might not have a group number.
How do I use my United Healthcare OTC card at Walmart?You can create a Walmart.com account and add your OTC Network card to your wallet or enter your OTC Network card as a payment method during check-out.
