Join Our In-Office Dental Plan!

At Reynolda Smiles we are pleased to offer an in-office dental benefit program for our patients who do not currently have dental coverage. This plan is similar to dental insurance, only smarter! The plan allows patients to receive optimal dental care while maintaining their oral health.

Annual Fees

Individual – $389/per year

Individual + One (Spouse/Child) – $730/per year

Each additional household member – $329/per year

What’s Included?

• 2 Regular Dental Cleanings (excluding periodontal procedures)
• 2 Dental Exams (periodic or comprehensive)
• 1 Emergency Exam
• X-rays, Bitewings (one per person per benefit year, excludes full mouth/panoramic x-rays)
• 1 PA X-ray
• Fluoride Treatment (one per benefit year up to age 19)
• 15 % Discount on general dentistry services (fillings, crowns, bridges, root canals, implants, etc.)

Plan Exclusions & Limitations

This program is not a discount plant, NOT a dental insurance plan and is secondary to any other dental plan. It cannot be used:

• In conjunction with another dental plan
• For services for injuries covered under workman’s compensation
• For treatment which, in the sole opinion of the treating dentist or doctor, lies outside the realm of their capability
• For referrals to specialists
• For costs of dental care which are covered under automobile medical claims.
• This plan is only honored at Reynolda Smiles Family Dentistry and can NOT be used at any other dental office.

Program Guidelines

• • You will not receive a membership card – your plan’s effective date will be on file with the receptionist.
  • Benefit coverage table is subject to revision annually.
  • There will be a $50 reinstatement fee if your plan lapses.
  • Cannot be used in conjunction with another dental plan.
  • NON-REFUNDABLE. No refunds or premiums will be issued at any time if participant decides not to utilize the dental plan.

• The Dual Plan is for parent/child or husband/wife only. The Dual Plan and Family Plan includes any child who is enrolled full-time in college until age 25, or a child who is not enrolled full-time in college until 18.
